Case: Hip Fracture (left leg)

Mr. Z is under respiratory treatment right now.

WBC(White blood cells) count elevated.

sZ, Mr. Z's son, talked to Dr. C to recommed Mr. Z to Dr. R, the head of ICU, for a blood test for blood counts and electrolisis.

Age: 82
Case: Hip Fracture (left leg)

Mr. Z is feeling much better today.

Alert and active.

Verbally responsive.

Still in ICU.

Friday Nov. 30, 2007

Patient's initial: A. Z.
Age: 82
Case: Hip Fracture (left leg)

Mr. Z's feeling better.

Little response from him.

Medications: through IVs

  1. Antibiotic
  2. Hepparin
  3. Metaprolol
  4. Beta Blocker

Noon Breathing better by the help of a breathing machine.

08.00pm Mr. Z can breath by himself without the aid of a breathing machine. Vital signs are being watched carefully.

Thursday Nov. 29, 2007

Patient's initial: A. Z.
Age: 82
Case: Hip Fracture (left leg)

Mr. Z has no reactions.

Alertness is very low.

Verbally unresponsive.

He was in a very critical condition.

He was using breathing machine.

Wednesday Nov. 28, 2007

Patient's initial: A. Z.
Age: 82
Case: Hip Fracture (left leg)

12:00am Urine output was small in amount. Mr. Z had hiccups every hour.

02:00am Urine output was medium in amount.

07:30am Dr. Y visited. Replaced Dr. U's medications. Mr. Z was very weak. Took oxygen.

11:30am Mr. Z was experiencing dyspnea (difficulty in breathing).

12:00nn Mr. Z was experiencing dysphagia (difficulty in swallowing) he was sent to ICU by Dr. S's group. He was not alert and response was minimal. He was breathing by breathing machine. All medication are through IVs.

Tuesday Nov. 27, 2007

Patient's initial: A. Z.
Age: 82
Case: Hip Fracture (left leg)

P.T. came. Mr. Z sat on the chair but cannot tolerate it. He experienced dizziness and nausea.

11.00am
Mr. Z had a chest and abdomen X-ray. Potassium and magnesium was replaced with a new one.

1.30pm
Mr. Z had a medication Lovenex for blood clot. Melaprolol for stool softener. Daoretic for nausea through IV.

3.00pm
P.T. came. Mr. Z did very well. Sat on the chair for two hours. His bowel movement (B.M.) was likely jelly in form.

Monday Nov. 26, 2007

Patient's initial: A. Z.
Age: 82
Case: Hip Fracture (left leg)

Medications:

  1. Antibiotic
  2. Metaprolol
  3. Diuretic
  4. Potacium
  5. Magnesium

Mr. Z had rashers on his back

11:00am The P.T. came. Mr. Z sat on the chair for 2 hours.

12:00nn Lunch:

  1. 1/2 soup
  2. 1/3 ice cream
  3. 1 tbsp. of castard sorbet

3.30pm Blood test. Mr. Z sat on the chair.

7:20pm Mr. Z threw up everything he had taken.
